I Participant, hereby acknowledge and agree to the following terms and conditions related to my participation in martial arts classes and activities at Cornerstone Martial Arts & Leadership Academy.

1. Assumption of Risk: I understand that participating in martial arts activities involves certain inherent risks, including but not limited to, the risk of injury, physical harm, and the transmission of communicable diseases. I voluntarily choose to participate in these activities with full knowledge of these risks.

2. Release of Liability: In consideration of being allowed to participate in martial arts classes and activities, I hereby release and discharge Cornerstone Martial Arts & Leadership Academy its instructors, staff, and affiliated organizations, from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in martial arts classes or activities. This includes, but is not limited to, any claims arising from negligence or misconduct.

3. Assumption of Responsibility: I understand that I am responsible for my own safety during martial arts classes and activities. I agree to follow all safety rules and instructions provided by the instructors and staff of Cornerstone Martial Arts & Leadership Academy.

4. Medical Treatment: In the event of injury or illness during my participation in martial arts classes or activities, I authorize Cornerstone Martial Arts & Leadership Academy to seek and administer any necessary medical treatment on my behalf. I understand that I will be responsible for any associated medical expenses.

5. Media Release: I grant Cornerstone Martial Arts & Leadership Academy the irrevocable right to use photographs, videos, or other media of me for promotional and marketing purposes without compensation.

6. Parent/Guardian (For Participants Under 18): If the participant is under the age of 18, the parent or legal guardian's signature below indicates their consent to the terms of this waiver on behalf of the minor participant.Form

I, the undersigned, hereby acknowledge that I have carefully read and understood the above liability waiver. By signing or submitting this form, I, as the participant or the parent/guardian of the participant (if applicable), agree to all the terms and conditions outlined in this waiver.
